I have seen the excel sheet of fixes for this. If the Event Codex and/or the Bestiary do not look properly loaded, enter a new game of any kind and once it loads up the town screen, press 'q' to reset the game and it should load the entries you have unlocked that were not properly unlocked.

I have played quite a few playthroughs of World of Horror and attempting to 100% whatever is possible at this time.
I periodically check the Codex and Bestiary and discover some entries have erased completely from tracking. I have Wiki'd those entries and confirmed them, even in the most recent playthroughs. The entries deleting seems random. What are your thoughts?
